The essence of expert psychotherapy is a real-time feedback loop. The psychotherapist continually adapts what they say and do based on the patient’s responses.
In this way, each patient teaches the therapist how to do psychotherapy *for them*
This is why real psychotherapy is not a reproducible procedure. It is treatment for one person alone.
Causation is neither linear nor one-directional. It is circular and reciprocal. Patient and psychotherapist mutually influence one another at all times. The success of the work depends on this.
Research trials on “evidence based therapy” begin with opposite assumptions. Exactly opposite assumptions. They assume causation is one-directional, from therapist interventions to outcomes. They destroy the reciprocal feedback loop by scripting interventions in advance. They proceed from the assumption that therapy can and should be standardized and repeatable.
None of these things are research findings. They are assumptions the researchers bring, before designing the research or collecting data. None of these things are requirements of scientific investigation. They are all assumptions, made by researchers who are not seasoned clinicians. And they are unexamined assumptions.
This is one reason I’ve said academic researchers are not studying psychotherapy. They are studying fictions of their own invention and *calling* it therapy.
It was a pleasure talking with @jf_moore for the @Mad_In_America podcast. I talk about my own and my family’s history of psychiatric hospitalization, the history of madness, stigma, psychiatry’s medical model, and its alternatives
Afternoon there's a surprise, the UK government has ignored mental health with the mental health bill not included in the #KingsSpeech, it’s now highly unlikely the Mental Health Act will be reformed until further notice I am not a politician but constantly over the years I have been told by people within politics "yes we need to do something yes there needs to be changes" I wish each one of the decision makers could spend a weekend with somebody who has mental Health issues
they need be there experience the heartbreak for the carer's go and also spend a night & day in a secure unit end of rant!
@MarkLRuffalo @UCDMedicine I gave a similar talk some years ago. Hope to access your talk. Are you linked to ISPS? We are discussing Transference and CounterTransference with respect to psychosis next week.
“If a thought has been really true for a day, its becoming false the next day does not deprive it of its claim to verity. It may have been only a short-lived truth, but if intelligent and serious it is worthy to be received, both artistically and philosophically.”
– C.P. Cavafy
In the 66 years of my life one of the most heartbreaking things I have had to watch is the Tories' systematic destruction of the NHS in order to replace a system dedicated to keeping us healthy with one dedicated to maximising profits. It fills me with despair and rage.
Has the @bbc actually analysed @GaryLineker’s tweet? Because it is factually accurate. A lot of the tropes used now by Govt: ‘invasion’, ‘menace’ etc, are pretty similar to those used by nationalist parties in 1930s Germany. Why no analysis? Are BBC history presenters muted too?
“Illness is neither an indulgence for which people have to pay, nor an offence for which they should be penalised, but a misfortune, the cost of which should be shared by the community.”
- Nye Bevan
