Rhythm changes during CPR: any impact on #ECPR outcome?
⚡️survival highest if persistent VF or conversion from initial non-shockable rhythm to VF
⚖️ survival lowest if conversion to or persistent non-shockable rhythm
⚖️ initial PEA & asystole? lower survival
🫀asystole at cannulation? worse outcomes + steepest survival decline with prolonged CPR
These specific survival patterns, rhythm-specific, time-dependent with increasing CPR duration, underscore need for individualized, rhythm-informed/time-adapted #ECMO decision-making algorithms to optimize outcomes.

🥅 goal of #ECPR should be survival with meaningful neurologic recovery: incentivization of cannulation solely for OD should be avoided
🥅 secondary contribution of #ECMO in cardiac arrest to OD increasingly recognized
Cumulative ECPR beneficence score remains elusive: personal, regional, ethical, interdisciplinary nuance at play; moreover, lived patient and family experience cannot be summarized through numeric quantification. However, consensus-based outcome metrics offer grounded framework for program evaluation: to be tracked OD metrics and survival outcomes with increased granularity.
