Why is Tracking left out of QA in most SW companies?

@usetwist Do you have any advice/tutorial on how topics should be defined? I'm wondering especially about the ideal level of granularity to both avoid noise/generic topics, but also avoid redundant and empty topics that need regular clean ups.
