🚨 Can a diabetes drug become a therapy for sarcopenia?
A new study in Journal of Cachexia, Sarcopenia and Muscle reports that imeglimin, a first-in-class mitochondrial-targeting antidiabetic agent, protects skeletal muscle from atrophy in both obesity and ageing mouse models. Rather than acting through classical anabolic pathways, imeglimin appears to suppress chronic inflammatory and oxidative stress programs within muscle.
🔬 Key findings:
• In HFD-induced obese mice, imeglimin increased the cross-sectional area of fast-twitch (Type IIX/D) EDL muscle fibers by 66% compared with untreated obese controls.
• In naturally aged mice, 12 weeks of treatment preserved EDL fiber size and improved grip strength despite no significant changes in systemic glucose tolerance.
• Transcriptomic analyses showed coordinated suppression of immediate-early genes, inflammatory pathways, and TGF-β-associated signaling, all molecular signatures linked to sarcopenia.
• In cultured C2C12 myotubes, imeglimin reduced palmitate-induced ROS production and inflammatory gene expression, suggesting a direct protective effect on myocytes.
💡 Why this matters
Current metabolic therapies—including GLP-1 receptor agonists and SGLT2 inhibitors—can reduce lean mass during weight loss, raising concerns about sarcopenia. In contrast, imeglimin demonstrated a distinct profile by preserving muscle while targeting mitochondrial stress responses rather than stimulating mTOR or other anabolic pathways. The data suggest that reducing redox-sensitive inflammatory signaling may represent a complementary strategy for maintaining muscle health during ageing and obesity.
⚠️ Important caveats
This remains a preclinical mouse study. Although previous clinical studies have reported improved muscle strength in patients with type 2 diabetes receiving imeglimin, definitive randomized trials evaluating muscle mass, function, and long-term outcomes in older adults are still needed.
