A tremendous and well-documented case of #SCAD by my friend @AugustoDaige. A 35-year-old female patient was admitted with STEMI and underwent fibrinolytic therapy. She presented with diffuse dissection of the right coronary artery (type 2) and a very characteristic OCT finding of a large crescent-shaped intramural hematoma. Note that there was no proximal or distal entry or exit tear, respectively. In addition, the hematoma separates the intima-media complex from the adventitia, which is exactly what is found in the histopathological findings. This reinforces the theory that the pathophysiology of SCAD occurs from the outside in. There is also no atherosclerosis. @AugustoDaige has contributed enormously to the Brazilian SCAD Registry #SCALIBUR. Congratulations on the beautiful case Augusto!
