طلبت من أحد كبار السن من مراجعي العيادة أن يُحضر أدويته في الزيارة القادمة.

عاد ومعه هذا الكيس.

كثرة العلاجات صادمة مقارنة بالخطة العلاجية المتفق عليها ،و الأخطر من ذلك: أدوية متعددة تحمل أسماء تجارية مختلفة، لكنها في الحقيقة لنفس المادة الفعالة أو من نفس المجموعة العلاجية، ومع ذلك كان يتناولها جميعاً.

عندها أصبح مفهوماً لماذا كانت قراءات الضغط والسكر، وبعض الأعراض، لا تسير وفق المنطق المتوقع من الجرعات المكتوبة في الوصفات.

في علاج كبار السن، لا يكفي أن نسأل: ما الأدوية المصروفة؟
السؤال الأهم هو: ما الذي يصل إلى فم المريض فعلاً كل صباح ومساء؟

هنا تتقاطع ثلاث مسؤوليات لا يجوز أن تعمل كل واحدة منها بمعزل عن الأخرى:

الطبيب عليه أن يراجع العلاج الفعلي، لا أن يبني قراره فقط على آخر وصفة أو آخر تقرير.

الصيدلي هو خط الدفاع الذكي قبل أن يغادر الدواء إلى البيت؛ يكتشف التكرار، ينتبه للتعارض، ويوضح الفرق بين الاسم التجاري والمادة العلمية.

والأسرة هي الضمان اليومي الأهم؛ ترتب العلب، تزيل المكرر والقديم، تراجع الجرعات، وتساعد كبير السن حتى لا يتحول الالتزام بالعلاج إلى التزام بخطأ متكرر.

كثير من المضاعفات لا تأتي من غياب العلاج، بل من ازدحام علاجات متشابهة بلا مراجعة واحدة جامعة.

نصيحتي لكل عائلة لديها والد أو والدة من كبار السن:

في كل زيارة طبية، أحضروا كل الأدوية كما هي، بكل علبها وأكياسها، حتى لو بدت كثيرة أو غير مرتبة.

فالقرار العلاجي الآمن لا يبدأ من وصفة جديدة دائماً؛
أحياناً يبدأ من تنظيف كيس الأدوية.
