Really heart warming to hear Dr Vicki Gallagher + Diane Williams talk about the PHMDP impact having 40% of their cohort having advanced care plan, 20% referral to social care, linking in with dementia support work. The ARRS funding enabled a care-coordinator to support @HTBNSSG
Had a brilliant breakout coffee session today where we heard more about the work the analytics team is doing to enable linked data to drive MDT thinking and working for critical cohorts across @BNSSG #PHM
I’m in the most amazing coffee breakout room where finance, clinical, managerial, commissioner have made the case for, built a team for and made a plan for getting GP costing data into strategic discussions about finance and economics. Took 15mins - watch this space @cheltk
Great thought and reflections PHM can help us develop a positive legacy from covid and ensure we focus on closing the gap and improving health inequalities. PHM a great convener to coalesce around a problem and shift the resources to drive better outcomes @HTBNSSG#PHM
