Kinetics of sevoflurane vaporized directly in #ECMO membrane lung 
🔬ex-vivo trials using circuits primed with Ringer’s lactate; sevoflurane vaporized at concentrations of 1–8% with SGF of 1–10 L/min. 
💨 sevoflurane effectively crossed PMP membranes, rapidly reaching reproducible peak concentrations to values consistent with clinical targets for deep sedation; clearance increased significantly with higher SGF 
⚠️ environmental concentrations well below occupational safety thresholds 
Delivering sevoflurane vaporized directly into ECMO circuits feasible/safe alternative sedative strategy. These results provide a robust foundation for translating this into pilot clinical trial aimed at optimizing ECMO sedation with inhaled sevoflurane in patients with severe ARDS.

🪦 successful weaning 48.3%, but high in-hospital mortality = 72.4%; late mortality 5.8% per 100 pt/y
🫘 AKI requiring CRRT 60.5%
🧠 neuro complications due to stroke 24.4%
🦵🏻 limb ischemia 7.6%
🩸 bleeding requiring reoperation 29.2%
#ECLS may be reasonable. Despite high morbidity and mortality (comparable with other PC-ECLS), myocardial recovery may be achieved with careful patient selection: young patients better candidates; pre-ECMO lactate peak appears strong predictor for mortality.

People often think the R radial sat will be the first site to show drop. This isn’t true.

The first vessel to experience hypoxemia It’s the coronary arteries.
