Kasbach-Merritt syndrome (KMS). also known as hemangioma- thrombocytopenia syndrome. It is a rare infantile vascular tumor that occurs in infants < 3 moths but may be seen in toddlers. Caused by kaposiform hemangioendothelioma and tufted angiomas. It has a triad of vascular tumors, consumptive thrombocytopenia, and hemorrhagic diathesis. Vascular tumors grow fast on the trunk, neck, upper and lower extremities, face, and retroperitoneum. There may be intracranial hemorrhage (ICH) and bleeding from internal vascular lesions. Vascular tumors are aggravated by interventions, infections, and trauma. KMS may be associated with giant-liver hemangioma. There is consumptive coagulopathy with hyperfibronolysis and severe decrease in clotting factors, like fibrinigen, causing life-threatening hemorrhage. KMS is associated with microangiopathic hemolytic anemia (MAHA), destruction of RBCs, and rapid progression to disseminated intravascular coagulopathy
(DIC) with high rates of mortality. Dx. Blood analysis, angiography, US, MRI, clotting factors, and tumor biopsy. Tx. Corticostroids to stop angiogenesis, platelet transfusions, fresh frozen plasma, and anti-coagulation medication. Some clinical studies showed that tranexamic acid, a fibrinolytic inhibitor, is safe and may be tolerated in infants. Embolization may be used. Surgeries and interventions must be avoided as they are dangerous and cause life-threatening hemorrhages.
