@kidney_boy If there are ongoing losses with K<3.3 I give 40meq for 2 or 3 doses. The next day the K tends to be normal or still low. You give a bit more KCl even if the K normalizes, if there are ongoing losses.

@captainchloride I've seen it twice, both with BUN over 200. Usually we dialyze for 2 hours with low blood flow but these 2 got full 4 hour treatments. One seized, the other very obtunded and lethargic.

@NavTangri @kidneydoc101 We systematically treat diabetics but nondiabetics, we take case by case. We are still using >300. I think the absolute benefit for 30-300 is low and might not outweigh costs/risks.

#askrenal I've seen warnings to not use #SGLT2i in kids <6 due to diapers. Has anyone tried them in adults with diapers? Did they get a fungal infection?
