CLOUD - Consortium for Local Ownership and Use of Data The green dots symbolize the dandelion. Like the seeds of a dandelion, the power of our data (and thus each of us as individuals) is only unleashed when spread. | General CLOUD info @ANewCLOUD
Reasonable Objective #1
Our national health information infrastructure enables any accredited medical end point, to share protected health information, on an appropriately identified patient in common, with any other accredited medical end point.
It should be as easy as selecting the other end point's identifier, selecting the patient in common, selecting the type of information to be sent, and pressing the send button.
Anything else, or more complicated, is unnecessary.
Health information technology vendors take note.
Judy Faulkner is the billionaire founder of Epic Systems, and a recent study by Dr. Christine Sinsky reveals the following time spent on Epic per 8 hours seeing patients:
• Infectious Disease 8.4 Hours
• Endocrinology 7.7 Hours
• Nephrology 7.5 Hours
• Primary Care 7.3 Hours
• Hematology 7.2 Hours
This is why Epic Systems has decreased productivity and caused burnout of physicians. Much of this time spent is pajama time and not reimbursed.
Physicians need to be reimbursed for all of the time spent on Epic.
If we want to increase patient access to care, we need to find an alternative to Epic Systems!

One of the biggest healthcare issues is patients don’t have all their health data in one place. So many siloed systems.
What if someone built a super app that pulled in all their data? They’d pay a monthly fee. Payors would push to their members.
Is anyone building this?
"...of genetic risk assessment in women with mutations in BRCA or other susceptibility gene." Out of every 100 people with #breastcancer, 5 cases are a rare subtype. Check out @MSKCancerCenter Rare Breast Cancer Center... fascinating work. 3/3 https://t.co/NWYRYDgjmd
Day 135/262 of my virtual "tour" of @BCRFcure funded researchers brings me to @MarkRobsonMD (@WeillCornell) "Most breast cancers are not the result of inherited mutations in cancer causing genes. For families with a high incidence of #breastcancer..." 1/3
"...however, a genetic component may underlie an inherited risk." Dr. Robson and his team "employ advanced technologies that incorporate information from genetic tests to enhance the precision... 2/3 #bcsm https://t.co/KpGw6OMXyZ
@SethGodinBlog today hits the nail on the head. "Building a bridge between now and later is often a challenge when we seek to rewire our tech." This is definitely true of #HIT and #EHR https://t.co/0WwBTsLW2u
Sometimes called “reasoning from first principles,” first principles thinking is the practice of questioning every assumption you think you know about a given problem, then creating new solutions from scratch.
@gabrieldane and, of course, there is the other strategy… skip past EHRs altogether. The answer to portable music wasn’t better batteries for CD players. It was a paradigm shit to the iPod and digital music.
Not a bad strategy. Give people a super-bad EHR, and then give them a slightly-less-bad EHR to make them happy…
…but wait …what if the EHR was actually good?
Day 122/250 of my "tour" of @BCRFcure funded researchers brings me to @Neil_Iyengar (@MSKCancerCenter) He's developed a digitized intervention. The Healthy Living Program offers individualized lifestyle prescriptions to improve #breastcancer outcomes. https://t.co/sSAIxXn6WV
As I return to my "tour" of @BCRFcure funded researchers, I am compelled to tweet once more on the enduring impact of @DrSusanLove One way we can honor Susan is her Love Research Army... to be sure all with #breastcancer have answers as unique as them. ❤️https://t.co/5pOoGCefIL
Day 108/250 of my "tour" of @BCRFcure funded researchers brings me to @Elizabeth_Comen work at @MSKCancerCenter who is "developing a minimally invasive blood test that can detect and classify early-stage #breastcancer and predict response to treatment."
Dr. Feinman at @lombardic aims "to identify novel microbiota-associated biomarkers that predict poor outcome which could lead to ... personalized microbial-based therapies to harness the immune system of #TNBC patients to ... defeat this disease." 2/2 https://t.co/NQAtjvrQiu
Day 101/250 of my "tour" of @BCRFcure funded researchers. Today's "visit" brings me to NJ. All research inspires me, but when it relates to #TNBC or #metastaticbreastcancer it always catches my eye. Maureen passed from pleural effusions due to this on 10.21.2014 #ASCO23 1/2
This still rings true. "Another thing that appears to have happened — especially with regard to the techno-sphere — is that the egalitarian & emancipating trappings of the Third Wave have been co-opted, commodified, or otherwise transfigured by Second Wave practices." 2/2 #ASCO23
Written in the 1980s, the Third Wave by Alvin (and Heidi) Toffler was prescient. It is one thing to be a futurist, quite another to be right. I wish they were both still alive to see what was once the future in the present. 1/2 #ASCO23
Steve didn't introduce a phone. "Ringtone" was just one of three functions on a reimagined device that we held in our hand. We have do what Steve did and reimagine the future of #healthit first then build a pathway from the #ehr to that new future. 2/2
Using a history of phones as a template, we are currently in the "princess" phone stage of evolution with #EHR development... we still have to have "ring tone" to connect to our data. When Steve announced the iPhone, he introduced it this way... 1/2
Diving into Super Pumped over the Thanksgiving weekend. Good show. Interesting to look back over the early days of Uber... step one... changing its name from ubercab to just Uber. To avoid regulatory capture. #LookingBackToLookForward
Imagine if email worked like social media right now... we'd need a different email address for every mail application. Twitter 2.0 needs to add new functionality... the rest needs to be like POP/IMAP where we can email/connect/tweet any old way we please.
