Should patients receive extended duration anticoagulation with reduced or full dose DOAC for the secondary prevention of recurrent VTE - The RENOVE RCT presented by @FCouturaud
Reduced dose DOAC was not non inferior to full dose ((5-year cumulative incidence 2.2% vs 1.8%, P=0.23 for noninferiority) but lower risk of clinically relevant bleeding (9.9 vs 15.2%)! 🩸
Risk benefit seems to favor reduced dosing for most patients #ASH24

Dr. @LeaderAvi provided a great review on initiation of anticoagulation in patients with brain metastatic disease. No difference between LMWH and DOACs and management needs to be tailored based on risk of ICH (risk factors and MRI findings)

Superficial vein thrombosis of the upper extremity are common in cancer patients. They are often managed conservatively.
@LaurenTokes presented her summer project at #ASH24 and showed that the risk of extension and recurrence is relatively high. This needs to be balance with bleeding 🩸
Many anticoagulant-related bleedings 🩸 in patients with cancer are attributable to modifiable risk factors. Addressing them (anemia, HTN, anti platelets, alcohol abuse) could improve outcomes in these patients!

1/ DOACs are preferred for prevention or treatment of thromboembolism in most patients with AF or VTE.
However, it should not be the standard treatment for patients with mechanical heart valves, atrial fibrillation with rheumatic mitral stenosis, or anti- phospholipid syndrome.
