The match took place on June 28, 1998, at WWF's King of the Ring event in Pittsburgh, Pennsylvania. What began as a planned stunt became one of the most infamous moments in professional wrestling history. After being thrown from the top of the 16-foot steel structure earlier in the match, Mick Foley (Mankind) climbed back to the roof. Moments later, the cage roof unexpectedly gave way beneath him, sending him crashing into the ring.

The fall knocked out several teeth, dislocated his jaw, caused multiple injuries, and left him with a concussion. Remarkably, Foley continued wrestling and finished the match. The event became a defining moment of wrestling's "Attitude Era" and is still regularly cited as one of the toughest performances in sports entertainment history.

The Undertaker, whose real name is Mark Calaway, later stated that he was genuinely concerned for Foley's life after the fall and feared the match had ended in tragedy.

Mick Foley has said he has only limited memories of parts of the match due to the concussion. Despite suffering injuries that would have ended most contests immediately, he not only continued but also endured a choke slam onto a pile of thumbtacks later in the same match. Many wrestling historians consider it the most physically punishing match ever performed in a major wrestling promotion.
