A new intern does a thorough history/physical which is supervised/double-checked by a senior resident which is then turned into an assessment/plan with help of an attending.

This case should be in the front page of every national newspaper. It SHOULD BE a sentinel case (like the LIBBY ZION) that results in a PATIENT PROTECTION LAW that requires SWEEPING REFORM in supervision requirements for NPs.

Error after error made in this case.
