EPoS II (NEJM interim): after high-risk adenoma removal, first surveillance at 3 vs 5 years. At five years, colorectal cancer was similar but marginally higher: 0.82% vs 0.77%. The later (5-year) arm meant nearly 4,200 fewer colonoscopies in that window.
Whatever the interval, the scope only sees what prep lets it see. A high-quality bowel preparation is how you get the most out of the exam, so small polyps aren’t left in the “might have been missed” pile.
