Fluid restriction in SIADH caused by subarachnoid hemorrhage (SAH) is generally A BAD idea.

🧠 Why this is tricky
After SAH, hyponatremia can be due to:
• SIADH
• Cerebral salt wasting (CSW)
They look similar in labs — but management is opposite.
⚠️ Why fluid restriction is dangerous in SAH
In SAH patients, maintaining euvolemia is critical.
Fluid restriction can cause:
• Hypovolemia
• ↓ Cerebral perfusion
• ↑ Risk of cerebral vasospasm
• ↑ Risk of delayed cerebral ischemia

👉 This can worsen neurologic outcome or cause infarction

🧪 Even if it’s true SIADH
Classic SIADH → fluid restriction
BUT in SAH:
• The brain is extremely sensitive to volume depletion
• Vasospasm risk outweighs benefit of restriction
So management changes.

💊 Preferred approach in SAH-related hyponatremia
• Avoid fluid restriction
• Maintain euvolemia or slight hypervolemia
• Use:
– Hypertonic saline (3%) if symptomatic
– Salt tablets
– Fludrocortisone (especially if CSW suspected)
🔥 Key distinction (exam gold)
• SIADH (non-neuro) → fluid restriction
• SAH-associated hyponatremia → NO fluid restriction
