@AOFAS_Journals I pretty much always take the metalwork out, in the belief it will optimise biomechanics across the midfoot and reduce the risk of complications from broken metalwork. A comparative study would be nice to prove the theory !
Summary in Bone & Joint 360 of our experience in Winchester running a trauma triage clinic. The rate of missed injuries was not zero, different to previous articles. Since then, GIRFT and transformation happened & trauma now centralised to Basingstoke. Have standards improved?

Should we remove metalwork after fixation of a Lisfranc injury? We have already shown there is national equipoise. Our NIHR application to run a pilot RCT is through to the 2nd round. Lots of support from the foot and ankle community. Let's hope NIHR agree!

@mskradiologist@togsTK@Jacobs_surgeon@WessexOrtho Good thought Neeraj. To stabilise the tendons I usually do a groove deepening by doing an osteotomy and elevating the posterior cortex to curette out the central bone, then tamping the flap down again. A burr and some bone wax also works well !

Patients who would normally be asked to nonweightbear for 6 weeks post op, are now being recruited to WAX. Hawthorne effect ?!

The reality for weekend warriors is very different and diagnosis of a high ankle sprain is often delayed, which prolongs disability. Look out for the posterior malleolus bone bruise on mri....sometimes the only indication of an unstable injury.
