Surprising that baseline tte only showed moderate TR (?undercalled). Preprocedural TEE looks like probable wide-open torrential TR (impressive laminar flow and likely rapid equalization of RV and RA pressure).

@NephroP @NephroGuy Do you have color+continuous doppler of the tricuspid valve (from that plax inflow)? Any concern of leaflet impingement due to lead? I'm assuming severe TR based on the hepatic vein systolic flow reversal which could be load-dependent from the LV but also wondering about the lead
