@amyfaithho what if they are not oriented to person, place, or time? What if they are just saying random words. Would you just put those random words in your HPI and move on instead of saying that the patient is a poor historian and history is obtained from family, etc?

@IdVilchez Could check a strongy Ab and if diarrhea a stool O&P (only about 20% sensitive though). Is the CSF also growing campylobacter?

@rampicillin @PulmCrit Yes. I guess avoiding a picc and hospitalization would be the main reason (that is if you had to use IV for some reason). 1x daily dosing through piv possible in outpatient infusion center if resistant to oral abx. Also depends on definition of “urosepsis”. ? Bacteremic.

I have a patient with suspected toxacara infection that had intitially presented with empyema/lung abscess that was culture negative. Positive serology for toxacara. WBC 19, 30% eosinophils in differential

@IdVilchez @shary_frank Probably not common enough to have an effect but azithomycin is better for legionella than doxycycline. I have found that with the new pneumonia PCR panel that legionella is probably a lot more common than i thought (negative urine Antigen tests with positive respiratory PCRs).

I have seen PCN resistance in Amp-S isolates. PCN continuous infusion is easier to dose outpatient than ampicillin which is not stable for 24 hrs. Frequent dosing of Amp can be prohibitive

Has pet pigs. Grew pasteurella aerogenes from sputum as well which can be associated with pigs. Suspect Balantidium coli. Parasites again seen on repeat BAL and sent to CDC. On Ceftriaxone/Doxy/Flagyl. Toxo serologies negative.
