@Soniabrwg The server wanted a 50% tip?! I’ve never understood the concept of tipping off of the total bill. Is it more work to serve a burger than a steak? $20 is more than enough
@drsanjaygupta monumental data released at ACC last weekend. Randomized trial = no difference in infarct by unloading the ❤️ after LAD STEMI. Both control & treatment arms had 31% residual infarct, thus the high ischemic HF rate. Only @ZOLLTherOx works but vastly underutilized

@MikketC@DFCapodanno@mmamas1973 But is the job really done when you still have a 31% residual infarct post stenting? TIMI III flow is myopically focused on epicardial patency without addressing the microvascular (90%) component of perfusion
@healthylifeMD Yet reopening the vessel quickly still results in massive infarcts (31%, both arms of DTU) which leads to poor outcomes. Supersaturated oxygen remains the only FDA approved therapy demonstrated to reduce infarct size in randomized trials against std of care stenting @ZOLLTherOx
STEMI DTU is discovery of what the cardiac surgeons have been saying for years: unloading the LV stops infarct evolution as infarct size was slightly smaller with Impella despite >40 min delay of PCI. Imagine a future technology that could do this in non-PCI centers. Imagine…

When spin trumps science!
Central hypothesis of STEMI-DTU: LV unloading will reduce infarct size.
Key results: Infarct size not ⬇️; AEs ⬆️.
Overall, benefit-risk not desirable.
JNJ PR: Infarct size not ⬆️ 🤷♂️

Dr. Navin Kapur and Dr. C. Michael Gibson discuss the STEMI-DTU trial: Mechanical LV unloading with the Impella CP device prior to primary PCI did not reduce infarct size compared with PCI alone among patients with anterior STEMI. #ACC26 View video here: https://t.co/TmocI68juw
