The one-page algorithm also lists important prescribing information including dosing instructions, possible side effects and monitoring recommendations
We also recommend more intensive treatment if the person is overweight or obese. That is, to start a GLP-1 agonist if the person meets the criteria according to the license if these medicines are available
3 core recommendations:
1: Co-commence metformin with high-risk antipsychotics (olanzapine,clozapine)
2: Co-commence metformin with medium risk antipsychotics + additional risk factors
3: Commence metformin with any antipsychotic if >3% increase in body weight in 1st year
Weight gain is one of the most common and distressing side-effects of antipsychotic medicines. But, co-commencement of metformin with can reduce the extent of weight gain by 4.03kg (95% CI -5.78kg to -2.28kg) compared to controls
This is the first published guideline for metformin for the prevention of weight gain. We used AGREE II & GRADE methods. International experts in endocrinology, metabolic psychiatry, pharmacy, general practice and patient & public partners came together to agree recommendations.
