Hepatojugular reflux is an exam finding seen in congestive heart failure.
The patient is positioned with the head of the bed elevated 30–45° and the neck relaxed so the internal jugular venous pulse is visible. Firm, continuous pressure is then applied over the right upper quadrant for 10–30 seconds while the examiner observes the jugular venous pressure.
A positive hepatojugular reflux is defined by a sustained rise in JVP of at least 3 cm that persists throughout abdominal compression and returns toward baseline after the pressure is released.
Sensitivity 12-24%, specificity 95-96%.

A simple way to see the 12-lead ECG
Color-coding helps pattern recognition:
🟢 Inferior: II, III, aVF
🟡 Lateral: I, aVL, V5–V6
🔵 Anterior/septal: V1–V4
🔴 Right: aVR (± V1)
Instead of memorizing 12 leads individually, think in territories.
This makes STEMI localization faster, cleaner, and harder to miss.
The resisted abduction test helps distinguish hip pathology from SI joint pain.
With the patient in the side lying position, the examiner passively abducts the affected hip approximately 30–45° while maintaining slight knee flexion. The patient is then instructed to actively abduct the leg against the examiner's resistance.
Reproduction of pain that localizes to the lower back or sacroiliac region suggests the SI joint as the likely source.
Sensitivity varies from 59-87%, specificity 87-100%.
