Had a disseminated Histoplasmosis case. Positive fungal stains on 2 different LN biopsies.PCR positive. Urine and blood Ag were negative. Repeat Urine Ag also negative. Wonder if it could be a regional issue since patient had recently immigrated from South America?
Any data on length of treatment for Acute retinal necrosis from HSV immunocompetent individuals? Thoughts on when/if to transition to PO acyclovir. Should decision to transition to PO from IV be driven by retinal exam?
#InternalMedicine Patient with Cirrhosis. Peritoneal Fluid with 600 Nucleated cells. 6% Neutrophil 14% Lymph, 80% Histiocytes. Any thoughts on Ddx/ explanation for such a high number of Histiocytes?

@camwolfe@OGdukeneurosurg@camwolfe Thanks Cam. The issue with looking at photos on a cell phone. At first glance thought that was a foley/ chyluria. Feeding tube makes sense and would be at the top of my list with with post op lymphatic leak being a consideration if this was from an effusion
@JuliaDHankins Very few cases of Autochthonous Chagas in the US. The one case that comes to mind was an infant in TN with multiple bites. Highly unlikely to aquite chagas outside of an endemic region. Could also get an entomologist to confirm identity…
