Dementia is littered with interventions like thickened liquids & PEG tubes that became standard of practice on theoretical benefits without clinical evidence. My editorial in @JAMAInternalMed gives 3 recs to consider if thinking about thickened liquids

"There is some evidence that some treatment programs actually make psychopathic offenders worse...insight-oriented programs help psychopaths develop better ways of manipulating, deceiving, and using people..but do little to help them understand themselves" https://t.co/Q8HnrTQuKs
"the usefulness of nirmatrelvir–ritonavir in patients who are not at high risk for severe Covid-19 has not been established."
Oops. Guess that should have happened before 20+ billion dollars were spent.
@dralicehan@DrToddLee@IdVilchez @JGPharmD @JonathanRyderMD Meropenem is not a good empiric drug for Strep pneumo! Worth remembering when teams are asking for meropenem for a sick CAP patient.
Less active than Ceftriaxone, PCN, BL/BLI!

SCHOOLS: Everyone admires transparency & honesty. Problem with this is that school closures were along political lines (not scientific) & press that favored Dems helped keep them closed including NYT. Chapter 5 of book I wrote #Endemic goes over this in detail. Super sad to see science politicized this much
Pandemic effect on widening inequality
"biggest effects..on v young children..their cognitive &language development & ... massive learning losses..correlated with.. extent..schools were closed”
"effects..will ripple out for many years, possibly..decades."
Civilizations are hierarchical. Hierarchy implies structure. Structure is necessary for functionality. Tyranny emerges in conditions of structurelessness.
A new study casts doubt on the CDC’s claims of an uncanny crisis in maternal mortality in the U.S., along with countless news articles to that effect. The study suggests that the CDC’s surveillance methods may be faulty:
“The high and rising rates of maternal mortality in the United States are a consequence of changes in maternal mortality surveillance, with reliance on the pregnancy checkbox leading to an increase in misclassified maternal deaths. Identifying maternal deaths by requiring mention of pregnancy among the multiple causes of death shows lower, stable maternal mortality rates and declines in maternal deaths from direct obstetrical causes.” https://t.co/oKmFWCNI0P
Future historians will note with incredulity that, in the midst of a pandemic -- when there should have been a premium on critical thinking and academic freedom -- a concerted effort was made to cancel @MartinKulldorff and discredit the Great Barrington Declaration.
This is an unacceptable situation. Taxpayers, in part, funded these vaccines, the FDA has required certain post-market studies, yet they are refusing to release results, telling journalists and citizens they have to FOIA them if they want answers.
A major review of randomized trials analyzing the role that exercise can play in combating depression: https://t.co/WCZkgQaQPK
Summary: Exercise combats depression, and more exercise combats it more. The effects can be (at least) as large as pills.
Please, look after yourself.

I know there has already been lots of chatter about this article. Just so much to love. #medicalreversal, surrogate endpoints leading us astray, shortcomings of mechanistic reasoning...

It’s that time of year again: Oxfam is making up data. Warning about ”widening and extreme inequality”, “Since 2020, the richest 5 men in the world have doubled their fortunes. During the same period, almost 5 billion people globally have become poorer.” The truth is different:
Large study of UK COVID19 app data:
🏠Household contacts made up 40% of transmissions
🕺"fleeting" contacts in the community = low risk of transmission
So, public health measures like outdoor restrictions & stay-at-home orders are difficult to justify

@davidcarr333 I don't know frankly if the hospital administration is prepared to take on anything more. Patients can renew at https://t.co/AfQYKhaajz while waiting if they have a smartphone (if the website is working:).
