@alexander_minh There have been 5 of these incidents in Gtr Manchester in the last 6 years. On 2 occasions the Coroner raised suspicions that all ward staff were fabricating observations.

@drcmday@ArturNadol7566 When training doctors we put a lot of emphasis on the importance of reflective learning and continuous improvement but the structure of NHS is designed to to crush any such attempts and destroy the person making them.
This important revelation by @ShaunLintern appears to show @NHSEngland acting in total contradiction of the principles of the ‘Hillsborough’ Bill, & I will raise this at Lords Second Reading on Sep 1.
