A 42-year-old woman presented with recurrent swelling at the left medial canthus 5 months postexcision of a presumed xanthelasma. The lesion initially suggested dacryocystitis; however, dacryo-computed tomography (CT) demonstrated a patent nasolacrimal system, and inflammation progressed despite administration of corticosteroids and antibiotics (A). Contrast-enhanced CT showed marked preseptal cellulitis without postseptal involvement (B). Skin biopsy revealed granulomatous inflammation with numerous intrahistiocytic amastigotes (Leishman bodies) highlighted by Giemsa and periodic acid-schiff stains (C). Polymerase chain reaction identified Leishmania donovani, a species classically associated with visceral rather than cutaneous leishmaniasis. The patient was treated with liposomal amphotericin B. Periocular cutaneous L. donovani is exceedingly rare and may mimic dacryocystitis. https://t.co/axY8bRY1UH #ophthalmology
