At that time, real scientists would obtain cancer specimens, meticulously study them, observe the specimens under a microscope for days on end and hand draw what they observed. Fungal spores, coccidia (intracellular parasites), etc. They understood how tumors grew.

Below are screenshots of a document from 1895 called "Micro-organism of Cancer". The focus of the document is fungus in its various forms and types. Protozoa and coccidia (the term used for intracellular parasites at that time) are also referenced. They understood that these were not human cells (or human cells that hadn't been invaded).
