In major trauma resuscitation we all think perfusion, bleeding, coagulation and temperature. BUT heart function may be pivotal.
Webinar with Prof Karim Brohi on the cutting edge thinking about myocardial resuscitation on Thursday evening @TraumaCareUK

Is YOUR trauma management causing refractory shock?😮
Are you unknowingly damaging your patient’s heart? 😮
Join us on Thursday evening and hear Prof Karim Brohi talk about why you just might be! (and what to do about it).

I think this every time I see media criticism that a rare disease has been “missed” in a patient presenting to ED with a common symptom.
Devastating for the individual, but a normal and totally expected outcome of the diagnostic process.W
Really important topic for Emergency Medicine from @CMO_England
Patients want certainty, but we give them probability.
Differential diagnosis should have probability attached, and we need to be better at communicating residual risk to patients.
EuSEM 2026. Large trial of chest pain 1hr v 3 hr pathway. No difference in ED stay - implying that timing of troponin result is not the rate limiting step in ED discharge.
Maybe future research needs to move the focus from a diagnostic to a system level intervention.
EuSEM 2026. Comparison of 1hr and 3 hr troponin testing in ?cardiac chest pain. Little difference in outcomes or ED stay.
Conclusion: Whatever your current practice probably little reason to change.
(Lancet meta-analysis in publication).
EuSEM 2026 conference - novel nerve agents.
Novichok poisoning - difficult diagnosis, but think if “wet opiate syndrome” ie patient looks like opiate overdose (coma, pinpoint pupils), but has “wet features” such as hypersalivation, sweating, diarrhoea or vomiting.
If the test is positive it doesn’t mean the patient has the disease.
If the test is negative it doesn’t mean that the patient is well.
It all depends on the pre-test probability.
Great talk from Wilhelm Berhinger at EuSEM 2026
In clinical reasoning debiasing strategies (slowing down, checklists etc) have very little evidence of success in avoiding cognitive bias.
Knowledge and practice effectively combat bias.
Laura Zwean at EuSEM 2026
At EuSEM 2026 contemplating that conferences need better filters on bad oral abstracts:
1) removed all failed procedures from the analysis to claim superiority of a method of shoulder reduction.
2) removed all cases with missing data to claim an association.
Basic errors.
Pelvic fracture management demands timely decisions from senior clinicians. This #TTLtips helps us understand priorities for IR, surgery and fixation while maintaining progress towards bleeding control.
