@javadm20 That’s is true, ECG is a guide not an absolute. Anatomy is always the most important factor. However this pvc looks like an LVOT SOO, based on the V2 transition ratio which seems to be consistent with a PVC transitioning in V3 of left side origin. @andresenriqueza@Ed_Gerst

Is mapping necessary when using PFA to treat AF? It definitely makes delivery of ablation lesions much more predictable and avoids narrow isthmus on posterior wall.

We have largely been in reverse - "learning by burning." having more mechanistic info would really drive progress faster IMHO. We have seen little new beyond PVI. #EHRAchat
