@ashjamesphysio@rcgp@thecsp@JonRyanCSP@RobYeldham@CSullivanCSP @KMiddleton60 @SHazzard@suehaywardgiles Reading this it’s hard to work out if it’s physio/MSK FCPs or generic first contact practitioners that are being discussed. The title doesn’t accurately reflect what (MSK) FCPs do, causes confusion to pts/GP staff, therefore pt care and pathways are affected. Name change needed
Following recent discussions on X we’d like to clarify that RCGP isn’t responsible for training course content or for keeping records of FCPs, but we’ll raise this in our next meeting with NHSE.
@jeehanlynch 2/2 many however are under skilled, under experienced and under supported. Doesn’t help that primary care services don’t understand the role/MSK pathway either. FCP best if run by local Trust/MSK. PCN-employed poorly integrated where I am
@jeehanlynch depends on what you mean by MSK specialist? I’d argue that to be a good FCP you’d need to be an MSK specialist. Do you mean APP-level, or quality rehab physio? You are right they are neither, but many have the appropriate skills to provide triage at interface service level 1/2
@ABAElliott @MRheumy Always get an X-ray for every single joint pain? Is that wasteful? How often would it change management? Or just best to do it before injection?
@thecsp@AbiHend A shambles. It’ll also impact on recruitment and retainment which is already challenging enough. And will the primary care hubs have to fund the training for the private providers of FCPs?
@MRheumy @Chopperchap@DerekGriffin86 Not sure many FCPs would be confident to diagnose and manage FM. I’m sure a lot of primary care/interface rheum referrals would be to facilitate diagnosis/rule out other conditions. Then it can be managed by GP/FCP without the need for secondary care.
@Physiozensam Thanks for sharing and great work by you. She’s lucky in a way to have seen you as it wasn’t a first contact appointment. Does your FCP service accept referrals from GP staff if they’ve investigated? We try to keep ours truly first contact. Thanks
Did you clap for the NHS during covid? If you did, now is the time to support nurses, doctors & other professionals who will soon to balloting for industrial action. Clapping didn’t pay any bills. Clapping doesn’t repay good will. Clapping doesn’t replace staff who are leaving.
@consultantPT Agreed. A few howevers off the top of my head -FCP is important if the rest of the pathway is good -most FCP pts should be acute so no rehab need -better to be ref with FCP diagnosis than GP -PT Depts don’t measure and show quality -very poor rehab is common
