In a stroke, anyone can see the bright spot on diffusion weighted imaging!

You set yourself apart if you can tell WHY it’s there!

1st step to find a stroke etiology is knowing the vascular territories & the borderzones

Can YOU remember the vascular territories?

Here's how!

➡️Vascular territories look a butterfly:
🔸ACA is the head/body
🔸PCA as the butt/tail
🔸MCA territories spread out like a butterfly wings.

➡️You can remember the organization because:
🔸The letter A looks like a head
🔸Letter P looks like a body w/a tail coming off the back
🔸Letter M sideways looks like butterfly wings

🌟Where body & wings meet are the borderzones!🌟

Of course, it’s more complicated than that.

▶️Medially, there are also small vessel territories—the lenticulostriates & anterior choroidal. These look like small legs coming off of the butterfly body underneath the wings

➡️MCA is by far the largest—just like butterfly wings are bigger than the body:
🔸MCA supplies about 50% of the supratentorial volume
🔸Covers lateral surface from the superior frontal sulcus to the inferior temporal gyrus

➡️ACA & PCA are much smaller
🔸Supply about 15% each
🔸ACA & PCA cover the medial surfaces anteriorly & posteriorly, respectively

🚨Borderzones are the remaining 20%🚨

Hopefully, now when asked about vascular territories, you can emerge from your cocoon!
