Duodenal biopsy in an HIV+ pt with chronic diarrhea:
🔬 Cryptococcus – variably sized (3.5 - 8 μm) encapsulated yeasts with narrow based budding highlighted by GMS & Mucicarmine special stains.
🧫 Cryptosporidium – tiny 2–5µm organisms attached to apical enterocyte surface.
A striking example of dual opportunistic infections in advanced immunosuppression.

🧫 Tissue cysts with bradyzoites ± tachyzoites may be found in Wharton’s jelly, amniotic epithelium, and villi.
🔬 Characteristic: diffuse lymphoplasmacytic or granulomatous villitis.
⚠️Often accompanied by hydrops fetalis & placentalis.

Adult granulosa cell tumor = most common ovarian sex cord–stromal tumor. Often estrogen-producing → risk of endometrial neoplasia. Histology: Call-Exner bodies + coffee-bean nuclei. Reticulin stain highlights nests/groups of cells, not individual cells.
