@perc_surgeon It depends...in my opinion a patient with a new 4th CCS angina is an unstable angina...and so I would perform the PCI of right coronary now and not wait 4 weeks.

I had seen various cases like this one, where the cabg failed for the high LDL levels. In this case I suggest stenting of LM-LAD, with the support of IVUS in order of valuing the aneurysm and measure the real size of Left main.

@FahdWaqar I have a similar case in a previous posterolateral infarct. In my opinion it is not a bridge but the effect of a large aneurism. MRI or CT to exclude a pseudoaneurism. Fabio
