"I think it is scandalous that the NHS is allowing the substitution of doctors by individuals without sufficient medical knowledge instead of the obvious response to doctor shortages: hiring more doctors." Dr Mel Ryan, BMA lead on preventing doctor substitution.
Advanced practitioners are being used to cover doctor rota gaps across the NHS. Read more: https://t.co/bwksW1sIqB
Us juvenile delinquents perform brain surgery for £43/hr, singlehandedly take care of multiple wards overnight for ~£23/hr, deliver babies in distress for £38/hr.
Oh wait, no, sorry... that's what we're asking for, we actually earn much less. And some of us don't have jobs at all.
So hey @wesstreeting, how about less bleating?
This is the cognitive dissonance that underlies the conflict.
On the one hand you can top up and do ED.
On the other doctors can't top up and do paramedicine.
It is a perverse situation where you can only see skill mixing as appropriate in one direction.
And this likely speaks to the fact that you may well be a very good paramedic where you can appreciate the skills and strengths required under the pressure, stress, and dynamism of the specific breadth of clinical decision making you may need to make in the clinical settings paramedics find themselves in.
But does this expose a lack of understanding or appreciation for the need of broad based medicine in a hospital setting? The access to wider investigations (and clinical reasoning for using them), the interpretation of data in clinical context, the ability to coherently consult with specialists, and/or the ability to communicate with GPs.
Performing an endoscopy without supervision does require consultant/SpR level knowledge & skills with extensive understanding of anatomy, physiology, pathology, management of pre, intra and post op complications. It is not just a simple task..
Dear NHS patients,
Whilst you’re waiting:
🚩 > 12 hours in A&E
🚩 Weeks for a GP appt
🚩 Months/(years!) for a hospital appt
Just know that there are >20,000 UNEMPLOYED doctors right now thanks to this government.
Physician associates who assist doctors in GP surgeries and hospitals will now be known as assistants.
Health Secretary Wes Streeting says this "better describes the role that they do".

Most doctors now have no confidence in the GMC.
So today we announce the BMA's NEW register for all doctors.

Trying to understand how we got to the point where we have to argue the idea that you need a medical degree to practice medicine
Not only that, trying to convince others that qualifications & regulation matter in a safety critical industry for patients
What do you even say 🤷🏻♂️
The GMC's duty of candour has still not been discharged. Key information has not yet been delivered to the Court.
This is essential. The judge want to understand GMC decision-making.

16:Wanted to be a doctor
26:Finished med school
28:Started core training
The career means I…
- Delay having kids
- Quit hobbies
- Miss many personal events
- Constantly work on exams+applications
Only to be told by some a 2 year PG degree is fine instead
Make it make sense
in 1858,the GMC was founded on the principle that patients "... should be enabled to distinguish qualified from unqualified practitioners".
They have failed in that duty.
In 2022, they failed Emily Chesterton and her family.
Tomorrow, they may fail you.
#NoMoreEmilys
The PA who totally misdiagnosed Emily Chesterton which caused her death IS still working at a different GP Surgery to do locum PA work because she refused to have tighter supervision!
Judge has recognised our case gets "right to the heart of the matter" and it may need to be extended for an extra day.
Key point our team made was that the decision by the GMC to not set scope was reached with no identifiable decision-making process. And this despite the GMCs own survey, echoed by many others expressing concerns over scope, consent and supervision.

The rest of the world looks at what is happening in the UK with Associates and shakes its head in despair. Now the World Medical Council has passed resolutions in the interest of UK patient safety
@medwma

Often I don't comment on twitter,
BUT the UK high court ruling today saying there is no confusion between doctors/physicians and PAs (Physician Associates) by misspelling on the first page @RCPhysicians calling them the Royal college of PAs is embarrassing beyond belief!
Nomenclature around MAPs causes such confusion that even official court transcripts contain basic errors regarding it
How on Earth are patients supposed to understand this?
Unfortunately, many do not
The consequences can be fatal
Multiple cases highlight this
@lengreview
We have published a report compiling the shocking testimonies of doctors who have witnessed unsafe practice by PAs and AAs. Patients are being let down by the repeated failures of NHS leaders to keep them safe.
