Not every breast cancer survivor needs the same intensity of follow-up.
Risk-stratify. De-escalate when appropriate. Intensify when risk is high.

T-DXd combinations may expand the playbook in HER2-low metastatic breast cancer.
DESTINY-Breast08, a phase Ib study, tested trastuzumab deruxtecan (T-DXd) with multiple partners in HER2-low mBC and showed encouraging early activity across several combinations.

• T-DXd + capecitabine: ORR 60%
• T-DXd + capivasertib: ORR 60%

• Capecitabine: 13.4 mo
• Capivasertib: 9.0 mo
• Anastrozole: 13.4 mo
• Fulvestrant: not reached at data cutoff
⚠️ Safety still matters
Grade 3+ AEs were common across cohorts, and adjudicated ILD/pneumonitis occurred in multiple arms, including a fatal event in the capecitabine cohort.
💡 Takeaway
This is not practice-changing yet, but it suggests that rational T-DXd combinations, especially with endocrine therapy or AKT inhibition, could be a major next step in HER2-low disease. Randomized validation is needed.

La meta quirúrgica ideal continúa siendo citorreducción completa a enfermedad macroscópica cero.
