@BirthTrauma Thank you. My daughter died in 2018 and CQC inspection from 2023 was inadequate but they always seem to slip under the radar. Good lawyers and PR sadly.

The PMRT is not fit for purpose. Allowing an institution to self report on circumstances where the outcome is a death is deeply flawed, especially when families cannot attend. Injustices will continue as long as this is all that some families are allowed.

@autinveniamviam@RMatthewsPsych Even with this scan we need to know what reference ranges hospitals are using - there is varibility between Trusts - some are seriously outdated
Selfishly hoping that MBRRACE is taken into account and @SWBHnhs is reviewed as the trust with worst recurrent ‘red’ record.
The 10 should focus on a cross section of maternity, not only high profile failures.
Avoidable deaths at every trust should be part of be a full PI.
