The PMRT is not fit for purpose. Allowing an institution to self report on circumstances where the outcome is a death is deeply flawed, especially when families cannot attend. Injustices will continue as long as this is all that some families are allowed.

@autinveniamviam @RMatthewsPsych Even with this scan we need to know what reference ranges hospitals are using - there is varibility between Trusts - some are seriously outdated

Selfishly hoping that MBRRACE is taken into account and @SWBHnhs is reviewed as the trust with worst recurrent ‘red’ record.

The 10 should focus on a cross section of maternity, not only high profile failures.

Avoidable deaths at every trust should be part of be a full PI.
