A notable shift in the new EULAR recommendations: imaging SpA is no longer just diagnostic.
MRI and ultrasound can help monitor activity, predict outcomes and guide treatment in SpA. #Rheumatology#EULAR2026

🦴 Spinal ligaments in #SpA: can ultrasound help?
This study developed and validated a landmark-based US protocol for thoracolumbar ligaments:
✅ Lateral costotransverse ligament
✅ Supraspinous ligament
✅ Iliolumbar ligament
In healthy controls, visualisation was feasible and reliability was moderate to high.
In patients with SpA, US detected inflammatory and structural entheseal lesions.
A new window for assessing axial inflammation beyond conventional imaging.

VEDOSS vs pre-#SystemicSclerosis: boundaries remain blurred.
This scoping review found major variability in how very early diagnosis of systemic sclerosis and pre-SSc are defined.
✅ 40 original articles included
✅ 3 definitions of pre-SSc identified
✅ 13 different applications of VEDOSS criteria
✅ Only 31.3% applied the complete VEDOSS definition
✅ Some studies included patients with visceral or fibrotic complications, challenging the “very early” concept
A consensus definition of pre-SSc is needed to support risk stratification and future preventive trials.

Inflammatory myopathies are typically associated with a myositis-specific autoantibody that determines the diagnosis and prognosis. Myositis subgroups have distinct pathomechanisms that now allow for targeted therapies.

🆕 Development and validation of SONAR-7: a novel ultrasound score for synovitis
Ultrasound is increasingly central to the evaluation of inflammatory arthritis, but feasibility and standardisation remain key challenges. This study introduces SONAR-7, a concise composite score designed for patients with psoriatic arthritis (PsA) and axial spondyloarthritis (axSpA).
🔍 What stands out:
Integrates greyscale and power Doppler findings across 14 joints
Demonstrates high specificity (95.1%) with good overall discrimination (AUC 0.83)
Shows consistent associations with:
Clinical disease activity (e.g. swollen joint count, DAPSA)
Enthesitis burden
Functional outcomes (HAQ, BASMI)
💡 Beyond inflammation, the inclusion of structural changes strengthens its clinical relevance, linking imaging findings with patient-reported and functional measures.
📊 Overall, SONAR-7 emerges as a pragmatic and time-efficient tool, with potential utility in both routine practice and research settings across the spondyloarthritis spectrum.
