@alisonleary1 The people our community nurses are caring for at end of life are often more complex too with multiple syringe drivers for symptom management. Our nurses want to care for people at home but increasingly pressured with resources.

The National diabetic foot audit data is in;
Our (Salford) diabetic foot ulcer outcomes are twice the UK average!
Not only have we reduced amputation prevalence by 48% in 6 years but for diabetic foot ulcers…
1. Time to first expert assessment (<14 days) is 80% (40% nationally)
2. Alive and Ulcer free at 12 weeks for severe ulcers is 60% (30% nationally)
We’ve shown you can see non-diabetic foot ulcers AND leg ulcers in a ‘diabetic’ foot clinic without compromising diabetes care.
We’re proud that our system change over the past few years is yielding such positive results. We’ve done all this within existing resource and not employed any extra staff.
How? We worked hard to listen, co-design change, build networks and address the barriers to change. We’ve improved care for 210 000 people and now expanding out to over a million through the Manchester Amputation Reduction Strategy (MARS)
