When referring patients to the heart failure team team, please make sure they have an echocadiogram available to view ( as recent as possible) an up to date BNP, U&E and albumin. Refer via medway. We will be your best friends 💕 @NUHMedicine

Happy first birthday to our Ambulatory day case unit! Many patients have avoided admission thanks to the availability of this service,it has been instrumental in giving people autonomy in their care.The number 1 advantage reported? Sleeping in their own bed 💕
