Thank you @CIHR_IRSC@SCC_CCS for this award which I don’t deserve so early in mid-career, but which I’m grateful for. Indebted to my family, team, collaborators, funders, & to 🇨🇦 for giving this immigrant a chance. So grateful to my nominators. Miss you, dad.. this one’s for you
📢 Congratulations to Dr. Harriette Van Spall, recipient of the 2022 CIHR-ICRH/CCS Mid-Career Lecturer Award in Cardiovascular Sciences!

🫀 Past, Present & Future of Diastolic Function – Key Takeaways
🔹 Diastolic assessment has evolved from invasive hemodynamics to a comprehensive #EchoFirst approach.
🔹 The 2025 ASE algorithm simplifies evaluation, reduces indeterminate cases (~0.2%), & improves accuracy for estimating LV filling pressures.
🔹 Age-adjusted e', E/e', PASP, LA strain, pulmonary vein flow, & IVRT are central to modern assessment.
🔹 The algorithm has been validated in 951 patients & adds diagnostic value for HFpEF beyond clinical scores.
🔹 Emerging tools—including LA chamber stiffness, shear-wave imaging, valve opening timing, & AI—may further transform diastology.

What makes a good biomarker for heart failure? Dr @hvanspall and Prof James Januzzi cut to the heart of it: we often force biomarkers to do something they weren't designed for. Watch here: https://t.co/hGTbXTDJTa

@ShariqShamimMD The history & physical exam are where I start. in absence of knowing what came up on these, I would start with basic labs, 12 lead ECG, & Echo to exclude common causes of her Sx and narrow the differential.
Myth: most patients newly presenting with HFrEF can't tolerate simultaneous, polypill, or rapid-sequence quadruple GDMT because of dangerous hypotension risk
Third PCI of the day, sub totally occluded lad. The important part of these missions is to empower the local Cardiologists to gain more experience and lead the cases.

#QuadTherapy for #HFrEF is essential.
🤔But is it enough?
🤔With new evidence, what is the current role for other #GDMTworks (vericiguat, digitalis)?

@DrJMarine If there wasn’t room for improvement, I wouldn’t have posted what I did & wouldn’t do any trial methodology work. But as someone who leads trials, I also know the standards that most uphold and the rigour with which most do their work. So it is easy for me to be balanced I guess.

@DrJMarine Papering over what? I posted about research misconduct. I have a vested interest & career-long commitment to improving trial conduct & reporting. You’ve posted the same opinion piece ⬆️ to make the same arguments over time. Please consider balance & proportionality, my friend 🤗.
@DrJMarine Shouting “medical research fraud”, as some do, at every instance doesn’t help. We work to improve trial, regulatory, reporting processes in addition to doing the work of trials. The latter adds important insights that guides the former; you really can’t do one well without other.
@DrJMarine Important for our posts to be balanced & evidence-informed.. better for credibility, but less likely to generate strong emotions. Trial processes are rigorous, & there are important insights that come from executing trials. 1/
@DrJMarine Somehow those decrying recklessness and massive fraud in one arena are very quiet when it comes to the other. All bad stuff to varying scales.
@DrJMarine Based on 2nd hand reports, appears that the research misconduct was apparent upfront & caused the financial loss. As for systems failing , compare what’s happening here to the massive, crypto crime & corruption that is ongoing… or handling of files on trafficked & abused girls.
@DrJMarine Appears that problem came to light because of the checks and balances. Quality assurance is a huge deal in trials. Things can be readily traced and tracked. Not sure how the pandemic affected audits. An independent analysis is underway.
PS does dishonesty ever make sense?
Dr. #MauraGillison, a surgeon and pioneer who discovered the link between #HPV & oropharyngeal cancers, changed her field. Born in 🇨🇦, she chased clinically impactful answers rather than recognition. A remarkable scientist and role model, gone too soon. https://t.co/kTiJk33ZKZ
