My biggest issues with Duke's answers

He is aware of the shortcomings, takes blame and wants to fix them. Great!

HOW do you fix them?

He offered no solutions and mostly said "we've done it before, so we can do it again" and "other teams have these issues too."

That inconsistency is a large part of the issue. You guys are all over the map with decisions and when you can't identify the causes and potential solutions, you'll continue to be inconsistent.

And yes, other teams have these issues, and when they do, people get fired and they start over.
