193 in-person attendees.
Physicians, APPs, surgeons, gastroenterologists, advanced endoscopists, motility experts and a room full of people willing to spend their Saturday thinking together about how we can better care for patients with foregut disease.
That was what I loved most about today. Not simply the education, but the thoughtful questions, different perspectives, debates, and exchange of ideas.
Complex foregut disease rarely fits neatly into one specialty or one algorithm, and today was a reminder of what happens when we bring those perspectives together.

The conference made it to closing remarks. The shoes did not.
Many first-year GI fellows across the country have celebrated a milestone over the past few weeks: their first trip to the cecum. 🍩
Shoutout to my people @IndujaNimma@allisonchin_ & #NihalKhan on reaching that milestone!
Reaching the cecum is an accomplishment. It reflects practice, patience, and persistence.
But here’s what I remind our fellows:
The cecum isn’t the finish line. It’s the beginning.
From this point forward, your growth isn’t just about getting there. It’s about becoming an exceptional endoscopist and physician.
As you work with different faculty, don’t focus on finding the “perfect” attending. Instead, leave every endoscopy list asking yourself five questions:
⭐️ What did they see that I didn’t?
⭐️ Why did they make that decision?
⭐️ What technical habit do I want to adopt?
⭐️ How did they communicate with the patient and the team?
⭐️ What would I do differently, and what evidence or experience supports that?
Every attending has strengths.
- Some are master technicians.
- Some are exceptional teachers.
- Some model clinical judgment, efficiency, communication, or professionalism in ways that deserve to be emulated.
If you leave each day having learned one new technical pearl, one new way of thinking, or one new habit to adopt, you’ll eventually develop something even more valuable than someone else’s style.
You’ll develop your own.
Congratulations to all of the first-year GI fellows reaching the cecum over these past few weeks.
Enjoy the donut but keep learning tomorrow.

High-yield discussions on:
• When to pursue genetic testing
• Managing CPUE and SPS
• Surveillance strategies for patients and first-degree relatives
• The expanding landscape of hereditary GI cancer syndromes beyond classic FAP and MUTYH
One of my favorite parts of hosting visiting professors is watching a room full of trainees, faculty, and community physicians all lean into the same conversation together. Education hits differently when it feels collaborative, practical, and clinically relevant.

Saturday night at @DDWMeeting our group went to dinner.
More and more people kept walking into the room, faculty, fellows, residents, mentors, collaborators, friends. And at one point I looked around and had one of those quiet full circle moments.
Not because success suddenly appeared overnight. Many people in this group have been putting in the work for years. Building programs. Publishing. Teaching. Leading. Showing up again and again.
But this year, seeing everyone together, you could feel the momentum.
