@craig_bwood Hi Craig. Within NHSE Children's Cancer service spec all PTCs need a late effects pathway including an end of treatment summary and individual care plan for every patient and transition to adult services. So it's something every centre should offer (or aspire to at least).

Tonight we celebrated with laughter & tears the life of my darling niece Phoebe.Whilst @BorisJohnson partied, she obeyed #lockdown#rules deeply missing f2f with loved ones. The isolation & sadness led to her #suicide 15mths ago
