This is so important. A conversation we have often in hospital, going home for EOL care needs to feel safe for everyone involved, bout just the patient. The max 3hrs a day of carers visiting is often just not enough for families to feel confident.

Yes this! Am currently doing a little audit of deaths within 3 days of admission to our dhb, finding many missed opportunities/signs of decline.
