HbA1c becomes inaccurate whenever RBC lifespan is shortened or altered, because HbA1c reflects average glucose over 3 months based on glycation of hemoglobin.
If RBCs don’t live a normal 120 days → the result is misleading.
🔥 HbA1c is inaccurate in these conditions:
1) Increased RBC turnover → falsely LOW HbA1c
Hemolytic anemia
Acute or chronic blood loss
Sickle cell disease
Thalassemia major
G6PD deficiency
Hypersplenism
After transfusion
2) Decreased RBC turnover → falsely HIGH HbA1c
Iron deficiency anemia
Vitamin B12 or folate deficiency
Asplenia
3) Hemoglobin variants
HbS, HbC, HbE, thalassemia → depending on assay, may give false values or unreadable results.
4) Conditions affecting glycation
Severe renal failure (carbamylated Hb)
Liver cirrhosis
Alcoholism
Chronic opioid use
High-dose salicylates
⭐ When HbA1c is unreliable, use:
Fructosamine (reflects 2–3 weeks)
Continuous glucose monitoring
Self-monitoring blood glucose

🔵 Fructosamine is formed when glucose binds to serum proteins, mainly albumin, in your blood.
🔵 Just like HbA1C reflects sugar attached to red blood cells, Fructosamine reflects sugar attached to proteins.
🔵 Since albumin has a shorter lifespan than red blood cells, Fructosamine reflects average blood glucose over the past 2–3 weeks.
🔵 It’s a great short-term indicator of blood sugar control & useful when quick changes are made in treatment or diet.
🔵 Especially useful during pregnancy, gestational diabetes or monitoring medication response.
🔵 Fructosamine is not affected by red blood cell lifespan - so it’s preferred when HbA1C may be unreliable (ex anaemia)
🔵 But Fructosamine can be falsely low in low protein states like liver disease or nephrotic syndrome.
