I want to talk about REMAP-CAP and the Oseltamivir data. I disagree with stopping of the trial [on the data that I have], and I think the reporting of this should have been accompanied with a preprint or more information. First, this is clearly an unexpected result +
🆕🔥🟢Post hoc subgroup analysis of the BALANCE RCT 7 Vs 14d duration of antibiotic therapy for bacteremia in immune compromised hosts
1037 immune compromised patients
No statistically significant mortality difference between groups with the upper bound of the confidence intervals being below the initial BALANCE trial’s 4% non-inferiority margin

Oseltamivir bei schwerer Influenza auf ITS mit höherer (19%) Mortalität als Vergeichsgruppe ohne antivirale Therapie (14%): erste REMAP-CAP Daten (!!!)
#CCR26 REMAP-CAP has been presented at our meeting a few times, and its always a pleasure to host the team and their results https://t.co/E5yIfY6j14
Today's presentation focusses on antiviral use in the influenza domain: Oseltamivir (5 / 10 days) vs. no antiviral

Hi all #IDXposts#IDTwitter peeps. For those of you who missed out on #ESCMIDglobal this year (or who went and want a summary), see the highlights of ESCMID Global 2026 from the CLARITY faculty https://t.co/4WeRBcH66F
@BradSpellberg@Factor_XII@ABsteward@DrEmilyMcD Yes I think this is what we should be doing
@costplusdrugs has vancomycin capsules for $1 each.
We have a network meta-analysis under peer review which shows that vancomycin taper is probably Superior to fidaxomycin at day 56
▶️What's new in Antimicrobial Susceptibility Testing (AST)/ EUCAST
🆕⚡New breakpoints: trimethoprim-sulfamethoxazole( Bactrim)
Previous breakpoints were higher than ECOFFs and bisect wild-type distribution
Similar to what happened with Steno!

New publication out: SAT in PJI!
🔎 Forty-two studies (n = 2524 patients)
• Practice varies (U.S. vs Europe)
• Success ~70–74%
✔️ Consensus definitions proposed (SAT vs EAT)

Important study!
🆕⚡⚡🔴Predicting oral cephalosporin susceptibility in Escherichia coli blood isolates using parenteral cephalosporin susceptibility testing
The authors aimed to determine whether AST for parenteral cefazolin, ceftriaxone or cefotaxime can serve as surrogates to predict susceptibility for oral cephalosporins(cefuroxime, cefaclor, cefprozil, cefdinir, cefpodoxime, cefixime)
Prediction of oral second-generation cephalosporins may warrant cefazolin MIC ≤2 mg/L, despite limitations regarding false resistance and susceptibility discordant.
Ceftriaxone/cefotaxime demonstrates a more reliable surrogate for predicting oral third-generation cephalosporins susceptibility https://t.co/8Nd6NC0bix
Because event rates are likely so low, it is unlikely @snap_trial will be able to recruit the 5000+ patients needed for oral switch mortality OR <1.2 [mainly because ratio scale]. But the trial is still valuable. What outcome would move the needle on EOS?

TAPER-V RCT
Initial Vancomycin Taper for the Prevention of Recurrent Clostridioides difficile Infection
In this randomized clinical trial of 256, a 4-week vancomycin pulse and taper regimen *had a probability of 74% to be superior to astandard 2-week pulse regimen in patients a first episode or first recurrence of CDI
*All patients received 14d pulse of vanco (standardized at 125 mg orally 4 times a day at the time of recruitment) →taper (125 mg q12h ×7d,then 125mg q24h ×7d) vs placebo for management of CDI (first episode or recurrence).
Vanco for 10d is obsolete! #IDXposts

🆕⚡🟢Effectiveness of Doxycycline in Combination with Other Antibiotics for Gram-Positive Periprosthetic Joint Infections: a Causal Inference Study
Overall, doxycycline was not significantly associated with treatment failure (ATE(IPTW) =-0.09; 95%-CI=-0.24 to 0.05; p=0.19). Subgroup analyses suggested that doxycycline reduced treatment failure by 23-26% in S. aureus infections (p<0.001), 25-28% in patients without fever (p<0.001), and 34-35% when both conditions were present (p<0.001) #IDXposts #OMWiki https://t.co/05cQkFV7sX
