We recently led an organization in a debriefing workshop.
Debriefing (or post mortem or after action review) after an event is absolutely critical for learning but its not always done well.
Here's are 5 key aspects we discussed.
1. Creation of a psychologically safe space
This is written by virtually the entire Emergency Department of Alberta's largest city. It is reasoned, thoughtful and most of all - absolutely correct. /2
INFO Debrief post-resusc has become the norm here in YYC.
Anyone on the team can request one, they are usually Charge RN led, and all those involved in the case are invited to join (MD, RN, RT, PCA, EMS, SW, learners, etc)
It helps to know we're not alone in the aftermath.

Follow up study 5 years after implementation of our clinical event debriefing (CED) process showed staff found that CED improved patient care and helped manage stress in the ED.
