@walinjom It's a VT, but the shape of the T wave in AVR makes me wander if there is a TCA comsuption history, if so I would consider bicarbonate, if not I woould shock the pt as this is a VT (regular rhythm rules out and AF with previous BBB) and the pt is haemodinamicably unsuccessful
