Vancomycin rarely causes antibody-mediated thrombocytopenia associated w/severe bleeding events. Usually, platelets drop by >90% from baseline after a median of 6 day-therapy. Pts w/a (+) antibody should have an allergy entered in the EHR & shouldn't be rechallenged w/vancomycin.
