I am an emergency psychiatrist who specialized in the tertiary care of the most severely mentally ill kids in my province.
I took a great pride in delivering excellent emergency care, such that I was very much a sought after physician in my tertiary hospital. I thought that would afford me the ability to influence the systems around me. I was mistaken.
Not only did my own health suffer eventually leading to burnout because of the lack of support services around emergency care, but my patients would often bounce between services, stuck in referral limbos or not qualifying for programs or being bent into programs that weren't a good fit because they were simply available and what the person needed was not.
Good psychiatric care in an emergency requires the following:
1) A COMPASSIONATE and SAFE space for someone to come in an emergency and feel cared for, such that they would want to come again in a future emergency.
2) COMPETENT emergency providers who have access to a range of services that will match their emergency assessment of what is needed. Not having avenues closed off due to beds being full or programs being waitlisted.
3) CONTINUITY OF CARE and handoff such that the physicians forward movement in the emergency setting can be carried forward by the next person rather than them being sent for another intake, another potential diagnosis and treatment direction, and confusion and lack of coordination.
4) CONTINUOUS CARE (literally, people that are available) such that the patient doesn't languish for weeks to months between seeing mental health professionals while and off is occurring.
5) AVAILABILITY of reassessment and the ability to convene multi-provider meetings when a patient's clinical situation changes, because the prescribed treatment might no longer be appropriate with new information or new symptoms.
6) A FUNCTIONAL MULTIDISCIPLINARY TEAM in which the disciplines involved have time to actually work with their patients and fulfil their professional goals with them
7) once a patient is known by a psychiatrist, a helpful, clarifying, and COMPETENT BIOPSYCHOSOCIAL FORMULATION of the patient should be generated in which the entirety of their experience and history that contributes to their current presentation is laid out, giving multiple targets for the team to address.
What we don't need in psychiatry:

❤️🩹 Her Majesty, who has been Patron of Emmaus UK for twenty years, visited the charity’s new centre for women. Residents, known as companions, shared with The Queen their experiences of overcoming homelessness and recovering from trauma.
Her Majesty also visited the on-site charity shop, a working forge where companions make jewellery to sell, and the new “Book Nook” supported by The Queen’s Reading Room.
